You must associate a new site with at least one prescriber to add it to the Janssen CarePath Portal. You will be able to associate multiple prescribers with this site after it has been created.
The National Provider Identifier (NPI) is a unique 10-digit identification number for health care providers. that remains with the provider regardless of location or job changes. The NPI must be used in all HIPAA standard transactions.
The State License Number (SLN) is a unique identification number issued to health care providers for a particular state. The number of digits in the SLN will vary by state.
